St. Josephs Quilt Event

The quilt ticket is the form that will be attached to your quilt at the Quilt Event.  We have included one example quilt ticket for you to reference and the actual quilt ticket form for you to fill in.  Please complete the quilt ticket form and then email or mail it with your registration.  In addition, PLEASE MAKE ANOTHER COPY & INCLUDE WITH YOUR QUILT when you drop it off.  The forms are below.  Simply double click on the links to view each form.  You can type within the form and print it out.
